Privacy Policy

MONARCH Staff will not disclose or discuss patient-specific Protected Health Information (PHI) that violates the privacy rights of our patients. Patients can expect that their privacy will be protected and that patient specific information will be released only to persons authorized by law or by the patient’s written consent. In an emergency situation when requested by an institution or physician that is treating the patient, the patient’s consent may not be required but a “call back” process to prevent any breaches of confidentiality verifies the name of the institution and the person requesting the information. PHI is defined as individually identifiable health information. All patient information will be kept strictly confidential. Any MONARCH staff or other person not directly involved in the patient’s care that need to access PHI must submit their request in writing to the privacy officer. The privacy officer will determine if they will be allowed access to the information.
